We keep things simple: we do not sell your data, we do not run ad tracking, and we do not use analytics software that monitors your behavior across the web.

We collect information only when you choose to share it with us:

• Contact form — When you send us a message, we collect your first name, last name, email address, phone number, plan of interest, and the content of your message. We ask that you not include personal medical information in this form; it is not a secure channel for protected health information, and a member of our team will contact you directly to discuss any health details.

We use this information solely to respond to your inquiry. We do not share it with third parties for marketing purposes.

Third-Party Services We Use

Our website relies on a small number of trusted third-party services to function. Each operates under its own privacy policy:

Tebra — Patient Portal

We use Tebra to power our patient portal. When you log in to your patient portal, you interact directly with Tebra's platform. Tebra may set cookies on your device to manage your session.

Google Forms — Contact Form

When you submit our contact form, the information you enter (your name, email, phone number, plan of interest, and message) is sent directly to a Google Form in our Google Workspace account. We receive it as a form response and use it only to follow up with you. Google processes this data on our behalf under a Business Associate Agreement.

Netlify — Website Hosting

Our website is hosted by Netlify. Netlify's servers deliver the pages you view and, like most web hosts, may log standard request information such as IP address and browser type. Netlify does not process our contact form.

Images, Fonts & Maps

Website images and fonts are served directly from our own website rather than from third-party image or font services. Our contact page includes a link to Google Maps for directions; Google only receives a request from your browser if you choose to click that link, at which point Google's own privacy policy applies (policies.google.com/privacy).

Other External Links

Our website includes links to Impact Health Sharing, Agape VA Solutions, and community partner websites. Those services receive a request from your browser only if you choose to follow a link. Their own privacy notices govern information they collect after you leave our website.

Cookies

Our own website does not set tracking or analytics cookies. However, the third-party services listed above (particularly Tebra) may set cookies on your device as part of their patient portal functionality. These are functional cookies necessary to make those services work — not advertising or behavioral tracking cookies.

You can manage or clear cookies at any time through your browser settings.

No Ad Tracking or Analytics

We do not use Google Analytics, Facebook Pixel, or any other behavioral tracking or advertising technology on this website. We have no interest in tracking your activity across the web.

Data Retention

Contact form submissions are stored in our Google Workspace account while we respond to the inquiry and for 90 days after the inquiry is resolved. Spam and duplicate submissions are deleted when identified.

If an inquiry results in a patient relationship, information needed for care may be transferred to our approved patient-record system and retained under our separate medical-record retention policy. The original website inquiry is deleted from Google Forms and any linked response sheet within 30 days after that transfer. Authorized staff review stored contact submissions monthly and delete records that have reached the end of their retention period.

We do not maintain a separate database of website visitor information.

Your Privacy Choices and Requests

You may ask us to provide access to, correct, or delete personal information submitted through the website contact form. Contact our Privacy Officer using the information below and identify your request as a "Website Privacy Request." We may need to verify your identity before acting on a request. We will respond as required by applicable law and may retain information when necessary to meet legal, medical-record, security, or dispute-resolution obligations.

Children's Privacy

Our website is not directed at children under the age of 13. We do not knowingly collect personal information from children. If you believe a child has submitted information through our site, please contact us and we will promptly remove it.

Changes to This Policy

We may update this policy from time to time. When we do, we will update the date at the top of this page. We encourage you to review it periodically. Continued use of our website after changes are posted constitutes your acceptance of the updated policy.
